Özet
2003 yılında hazırlanan Sağlıkta Dönüşüm Programı ile sağlık sektöründe köklü değişiklikler yaşanmış olup süreç halen devam etmektedir. Sağlıkta Dönüşüm Programı, hizmet sunumundan finansmanına, sağlık personeli istihdam biçimlerinden yönetsel yapıya kadar sistemi bütünüyle değiştirmektedir. Birinci basamak sağlık hizmetleri sunumunda da, 1961 yılında Sosyalizasyon Yasası ile kurulmuş olan sağlık ocağı sisteminin yerine aile hekimliği uygulamasına geçilmiş olması da yaşanan bu değişimlerden biridir. 2005 yılında Düzce ilinde pilot uygulama olarak başlayan aile hekimliği uygulamasına kademeli olarak geçiş yapılarak 2010 yılı sonunda bütün iller uygulamaya dâhil edilmiştir. Aile hekimliği uygulaması ile birinci basamak sağlık hizmetleri sunum ve kapsamında değişikler yapılırken; birinci basamak sağlık hizmetlerinin sunum sürecinde yer alan sağlık personelinin de çalışma koşulları değiştirilmiş ve sağlık personeli sözleşmeli statüde görev yapmaya başlamıştır. Çalışmanın temel konusunu, aile hekimliği uygulaması ile birinci basamak sağlık hizmetlerinde yaşanan dönüşümün ortaya konulması ve bu doğrultuda aile hekimliği uygulamasında görev yapan sağlık personelinin, uygulamaya yönelik görüşleri, bakış açıları, çalışma koşulları ve çalışma süreçlerinin değerlendirilmesi oluşturmaktadır. Araştırmanın evrenini, İzmir ili Konak ilçesinde aile sağlığı merkezlerinde çalışan 248 sağlık personeli oluşturmaktadır. Söz konusu evrenden 165 çalışan araştırmaya dâhil edilmiştir. Araştırma verilerinin toplanmasında "Sağlık Çalışanlarının Aile Hekimliği Uygulamasını Değerlendirme" ölçeği kullanılmıştır. Araştırmada elde edilen veriler SPSS programı kullanılarak analiz edilmiştir.
Health sector has radically been transformed by means of Health Transformation Program which was implemented in 2003, and this process is still in progress. Health Transformation Program completely changes it from service delivery to finance as well as from ways of employing health care staff to administrative structure. One of the changes is Family Medicine Practice, which was put into effect with a view to delivering primary health care service, instead of community clinic system, which was practised as a result of the Socialization Law of 1961. Family Medicine System which was implemented as a pilot project in Düzce in 2005 gradually came into effect and at the end of 2010 all the cities of Turkey were included in the project. Along with the fact that primary health care delivery and its extension became modified as a result of Family Health Practice, the working conditions of health care staff in the process of primary health care delivery have been modified and health care staff have begun to work on contract. The purpose of this study is to determine the transformation in primary health care service as a result of Family Medicine Practice and to assess the working conditions of health care staff serving in Family Medicine Practice as well as their point of view of Family Medicine Practice and the working process. The research consists of 248 health care staff who have been working in family health centers in the district of Konak in the city of İzmir. 165 health care staff were included in the research. The Assessment of Family Medicine Practice by Health Care Staff scale was used to gather data for the research. The data collected in the research were analysed by using SPSS.
